A New Jersey Department of Health and Senior Services Office of
Emergency
Medical Services (OEMS) EMT Training Fund Certificate of Eligibility
for
EMT-Basic
or Continuing
Education Courses can be accepted in lieu of payment for the
courses
listed
below.
- Applicants must be
an active member of an eligible
NJ volunteer ambulance agency, according to OEMS.
- Students using the NJ EMT Training
Fund for
course payment MUST mail both a completed registration
form and a Certificate of Eligibility for a Continuing Education
Course (available in Adobe .pdf format
or Microsoft
Word format) to the above address.
- A separate training fund form must be provided for each
course. One form cannot be used for multiple courses.
- The training fund eligibility form must:
- carry an original signature in pen (Copies or faxes of signed
forms are not
acceptable. We must submit the original to OEMS.)
- signed by a principal officer of the volunteer first aid or
rescue squad (ie. Captian/Chief, President, Treasurer) other than the
student applying for the course (i.e., squad officers cannot sign their
own forms).
- In the event of incomplete coursework, failure, or absence (i.e.,
CEU credit is not awarded) the student will be responsible for full
tuition and billed according to our cancellation policy. Students
with an outstanding balance will have other current course
registrations withdrawn and not be permitted to register for another
course using the EMT Training Fund until full payment is made.
